Referrals to Home and Beyond Occupational Therapy are accepted from any source, such as clients, family, GP’s, or support coordinators. Please email or call first to discuss your specific requirements, then complete and email the below referral form:
Fees & Funding
Services from Home and Beyond Occupational Therapy may be funded in the following ways:
- NDIS
- Home Care Packages
- Medicare / Chronic Disease Management Program
- Private / self-funded
- Private health insurance (dependent on your level of cover)
